thats why i asked, it never seemed to work for me like that, maybe you need to change the units attack target allowed to mimic that of a meatwagon.

And make it be able to target ground and have splash so it will create in the splashed area the plague

Yes, the Disease Cloud (Meat Wagon) ability only works for units with Artillery attack type.
