Hero icon doubled on revival screen

Hero icon doubled instead on revive screen

Okay. So. I have a problem. My map has an alter and when a hero dies, it's icon shows up twice. Any ideas how to change it back to normal?

Uhhh... Do you have two heroes? Is one of the icons to train that type of hero?
