How do I do a quiz?

Hello every one! I´m making a map, and want to have a quiz in it... I dont realy know how I´m going to do it... I want that every player will get 2questions and if they has wrong they will die, if they has right they will survive to the 2nd question, if they has right on all of the question they will go on to next "level"... If some one can help me I would be realy happy!

Yes like that one.. I didn't remember where I played it.. But I can have the dialog instead...

One thing I dont understand is how I´m going to do to kill the one who not answer right...
